Welcome and thank you for choosing Dr. Christoforetti as your Orthopedic and Sports Medicine Specialist. Our practice mission is to provide excellent service and to improve the standard of care for people in our community.
We want your office visit to be as smooth and efficient as possible. In order to ensure that you are seen in a timely manner, we recommend the following:
- Fill out Patient Forms (found below) prior to your appointment
- Bring the following information to your appointment:
- Driver’s License or a valid ID
- Insurance Card
- Referral Letter (if required)
- X-rays, MRI, CT scans, etc. and/or any other relevant information (Images must be brought in on a Disc)
- List of medications (if any)
Please plan on your visit lasting a minimum of 1-hour and understand that additional x-rays, MRIs, and/or other testing may be required.
